AHA BP Tracker
The American Heart Association (AHA)'s BP Tracker is an integral component of the SPONSORED version of AHA's Check. Change. Control. (CCC) High Blood Pressure Management program.
Through a combination of remote BP monitoring and health mentoring, the evidence-based CCC program has shown proven results in helping people manage risk factors that lead to cardiovascular disease and stroke.
The AHA BP Tracker receives and displays diastolic and systolic blood pressure measurements, as well as heart rate data, from A&D Medical's family of wireless blood pressure monitors including their multi-user BP kiosks (for use in clinics, community centers, offices, etc.) and personal BP monitors (for at home, on-the-go, and remote patient monitoring).

Shouldn't be in the Canadian Play Store. Hard to use compared to alternatives here. Requires a campaign code to create an account. No legit codes available outside the US. Data entry UI is primitive. Disappointing.
Update: developers provided a password that makes the app available to all users; therefore I felt it unfair to leave my original 1-star rating. I've added 2 until I've used it enough to properly evaluate it again but, based on the most excellent website and twice daily text reminders, it's quite likely I'll increase the stars to 5 in the near future. Thanks to developers for their clear and prompt response.
Won't let me enter my phone number. Either of them. My device number = no. I also have GoogleVoice = also no. So am I using this app = no.
why is this app even offered? it requires an activation code but wont give you an activation code.

You need a code to start using the app and there's no place where it directs you to go for one. It won't recognize my online profile.
I have my moms account and I wanted to used it on this app.... But says that her email isn't valid? Register again.... And doesn't accept the same code I used in the main page 😕

I have an account online but it will not let me sign in using this app. Its not of much use of I cannot access it from my phone.

I opened up the CCC tracker on my computer desktop, and it worked just fine. I installed the tracker on my phone and everytime I try to login he gives me an error message of invalid credentials. I have been able to login successfully on my laptop, but I can't seem to get the damn thing to work on my phone
