Epicollect5 Data Collection
It provides both the web and mobile application for the generation of forms (questionnaires) and freely hosted project websites for data collection.
Data are collected (including GPS and media) using multiple devices and all data can be viewed on a central server (via map, tables, charts).
Data can be exported in CSV and JSON format

I've been using this app for data collection to support a mapping hobby. I'm very pleased with the ease of set up and with the downloadable database. It's easy to use in the field and has been perfectly reliable for my use. Thank's for making such a well designed and maintained tool available for widespread use.
Very goos. This is easy to make forms and to collect data. Transferring to Excel is not very easy when Indic scripts are used. That needs improvement.
Useful app for data collection even in offline....no need of Excel entry... reduce the burden of data entry and fotos available

Very useful app...in easy data collection..laptop and mobile friendly..but why not in tablets too...it shows application not compatible..please do something for that

Great app for data collection. My only trouble is I'm unable to enter the seconds while setting the time which is crucial for me. Please update the same

This app is getting famous in PGIMER, Chandigarh. I also have been using it for 2-3 months now for my thesis. It is a good app much better than data collection on paper!
very good app, only drawback as far as I Am concerned is that you cannot see the entries of other user of your project in your mobile, plz add the option to see all entries through mobile app done by your project members

Nice work team Epicollect! Creating basic forms was straight forward. I have some suggestions which would rank this app an easy 5 stars from me: 1. Exporting pictures - there must be a easy way to export them in a labelled fashion. Other apps such as fulcrum allow batch export as a zip file (this would put your app well and truly above all other data collecting apps out there); 2. Allow 'hiding' of none essential questions. Thanks in advance for your support.
