The update seems to have improved the app. This is the closest thing to what I want but it would be nice if you could change the font size. My 2 page document on paper turns into 16 pages in the app. Also some of the breaks in the page cut words in half if that's where the page breaks

A great app, for a while. I created a service form and had submitted about 30 completed forms via email. During my last form filling the app froze, then all my previous forms disappeared and so did the form itself. I'm afraid I can no longer trust this app.

Good app. Easy to use. It'll be better if you add a field for number such phone numbers. And ability to zoom in and out picture field to crop them. Thanks.

This app is exactly what i was looking for for months. It gives the abilty to create a form with desired layout and store & share information. Home run, thanks for the app !

I'd give it 5 stars because it's well layed out, but the problem is that there's no option to print out or share and also the logo field doesn't allow you to upload an image 😔

The app won't save or won't work beyond two field in the form, after submitting a 10 question form they were gone, after trying again, it will stuck on save/upload forever.

Everytime you press save it freezes up my score is a 0 stars .but to be able to post this I have to rate it.

Can't use the app if not create an account with them first. Really? Insane! Deleted rush.

Hi after sending files its not aligned wells and too many bugs while creating forms

When added option field, and trying to save the form, it's hanging..

Even lack something, analyze data in form, but the app was good

Pretty cool app but the app didnt seems to start at several instances recently
