NYSOH Mobile Upload

Did not perform its core function. I logged in (irritating that it does not retain the login info) and took photos. After that I clicked "submit" and it failed without detail. I tried it on another phone and this time after photos, the "submit" button never appeared. This "first version" isn't ready. BTW, the "star rating" isn't accurate. It SAYS 3 star but the average of the reviews is 1.72... interesting. Too much time on my hands I see.

It worked well. However I don't understand why we need to download and install an app to upload documents when it can be done through the online account 🤷‍♂️

This app is horrible because it takes dark pictures and doesn't want to submit it. The app should allow documents to be downloaded from the mobile or android devices too since it requests access to them. If you have a problem submitting the documents, you will have to play with an automated telephone number to actually receive assistance versus a person who can help you with the situation immediately. I was surprised to get a good person to help me.

I have been trying to upload a document. The app shuts down as soon as I try to take a photo or pick a photo. It's so frustrating! I will try again tomorrow.

Must call to find out submission failure is a result of sending more than one photo at a time. A note to that effect....that, "if submission failed, try one photo at a time".... would save much time, and waiting until call center opens again Emphasis on the acceptance of ONLY two items of ID verification, would stop some folk from attempting 3 items (name, dob, ss#).....wasting time attempting to input 3rd item

Oh my goddess! Is this app has the most unwieldy camera function I have ever used. When I take a photo in manual mode I don't expect the app to crop my photos! The stupid camera keeps chopping the top off the forms. After trial and error and multiple redoing of it even after I had figured it out it still was a chore to use the camera function. As for the automatic camera, I'd still probably be waiting here for it to take its first picture if I tried using it again. Too slow!

It WORKS! I'm sick and it is not easy to submit documents. I just downloaded app, followed instructions, and submitted docs. I even received a confirmation of submission on my email. Easy, peasy! It is space sensitive. Do not leave space after name or email if you want it to work. Thanks, NYSOH! It works, and I'm grateful.

I had to use this app to submit documents. It's very user friendly and I submitted my documents with ease (4 photos). When I logged in for the first time, I was locked out of my account. The message says I tried more than 5 times to log in. Not sure why that happened but I waited a few days and was able to log in.

DO NOT USE THIS APP! It was easy enought to upload the documents. After uploading, the app asks if you want to finish or upload more documents, i clicks finsh, and it sends me back to uploading documents. If tpu go into the menu tap and click exit, it sends you to upload documents, and when you finally force the exit bu swiping away the window, you get an email the the document was not uploaded. THIS HAPPENS ON A LOOP.

Very easy to use. I wish the app also uploaded pics from one's gallery so that you can upload a document when you don't have it in hand. Also would be great if one could add a comment to uploads.

...I could only upload 1 photo at a time when it clearly provides the option to be able to upload the 4 I needed to upload at one time. After figuring that glitch out, I uploaded 1 photo at a time. I had to use manual mode also. Sign in was simple. Make sure you scroll to the bottom of the terms to get to the next step.

Only option provided is photo. There should be an option to access documents on your phone. Everyone doesn't have access to a printer. App should be more mobile friendly.

Surprisingly worked great. Didn't feel like calling in to the customer service line, and took a chance that I'd be able to submit documents without them being requested, and so far looks like it worked.

It would be great if you could attach documents not onky take a picture pf them. I can't print things out, and sometimes the picture of the monitor is pixelated.

Great app easy to use, once you complete the account setup/login you can begin sending in documents and it only takes a few minutes. Quick and Simple.

App works and is fairly straightforward, allowing us to quickly submit documents to the New York State of Health. We had trouble with the automatic camera function but the manual function still works well.

I have no option except to take a photograph of any documentation that I need to upload. This is an inconvenience. The app should allow me to upload downloaded documents from my phone. It asks for my permission to access my camera. It should request information to access documents on my phone.

This is the best and easiest app for submitting and uploading documents. I certainly wish all NY Government Agencies and/or agencies period, had an app like this..especially now in this day and age (COVID19) to avoid being in crowds of people.

Needs to give the option to upload pdf files that are downloaded on phone. I had to upload a document and it would not give me the option to upload something I downloaded into my phone. I had to wait a few days until I got access to a printer to print it out and take a picture of it to upload. Very inconvenient and resulted in my document from being submitted late.

Hard to get socuments to submit. Submitted documents to update my premiums a few months ago and never got increased payment. No email, no mail, cwlling didnt help. Now my premiums are up again, well see if this actually works this time.

I have tried many times to submit documents and it always says wasn't able to upload the photos. Terrible app and also the nystate of health doesn't work correctly, impossible login.

Easy to use once the internet/site is working. I tried a few times before being brought to the screen to submit my documents.

This is a really great hat but the only problem with the photo upload is that it only allows you to take pictures it doesn't allow you to upload from the gallery or from the genius scan scanner

Extremely helpful app. It helped me to submit documents that are needed to update my account. I am glad that the representative at NYSOH I spoke to informed me if this app. I will use it for any documents that need to be submitted.

App will not accept any more than 3 photos at a time. Very aggravating when you have multiple photos that need to be submitted.

Very easy,unintimidating for someone like me who is still catching up with technology. Now,if all goes well,and everything is sent and received,I would have no problem changing this to 5 stars.

This is a beautiful way to do things the right way I am surprise how good this app works real easy to understand real easy to take the photos and all your document they ask for hey man you got a 10 star from me... Way to go man good job good job guys on the app..

Absolutely useless.. re-logging in each time you have to do anything.. not much in the way of support.. pretty standard for NYS
